Objective: Clozapine is a second-generation antipsychotic indicated for treatment-resistant schizophrenia. Studies in several countries have shown a low rate of clozapine use despite the fact that approximately 30% of schizophrenia cases are treatment-resistant. In Brazil, few studies have addressed the frequency and variety of antipsychotic use in individuals diagnosed with schizophrenia (ICD F20). The objective of this study was to measure the rates of clozapine use in this population in the last decade using Brazilian Ministry of Health data. Methods: Prescriptions made between 2010 and 2020 in all 26 states and the Federal District registered at the Outpatient Information System Database from the Brazilian Health System (SIASUS) were evaluated. Results: A total of 25,143,524 prescriptions were recorded in this period, with clozapine representing 8.86% of all antipsychotics. The most frequently prescribed antipsychotic for patients with schizophrenia was olanzapine (35.8%), followed by quetiapine (27.5%). From 2010 to 2020, the rate of clozapine prescriptions in Brazil increased from 7.2% to 10.9%. Conclusions: Despite a slight increase in prescriptions in the last decade, clozapine is still underutilized in Brazil.

Objective: Clozapine is underprescribed due to neutropenia risk. Blood tests every 3 months in those on continuous treatment for > 1 year who have never had an absolute neutrophil count (ANC) < 2,000/µL has been proposed as a monitoring strategy; however, there are no South American data to support this recommendation. This study sought to investigate whether clozapine use and other variables could explain the occurrence of ANC < 1,000/µL in patients with severe mental disorders. Methods: A total of 5,847 subjects were included, 1,038 on clozapine. We performed a Cox regression considering the outcome as ANC < 1,000/µL at any time point. Predictors were sex, age, ethnicity, clozapine use, ANC > 2,000/µL during the first year of blood monitoring, and presence of a severe medical condition. Results: In the Cox regression model, ethnicity (white) (hazard ratio [HR] 0.53; 95%CI 0.29-0.99, p < 0.05) and ANC > 2,000/µL (HR 0.04; 95%CI 0.01-0.10, p < 0.001) were protective factors, while presence of a severe medical condition (HR 69.35; 95%CI 37.45-128.44, p < 0.001) was a risk factor for ANC < 1,000/µL. Other variables were not significant, including clozapine use (HR 1.33; 95%CI 0.74-2.39, p > 0.05). Conclusions: These findings suggest that clozapine does not increase the risk of neutropenia in subjects with ANC > 2,000/µL during the first year of use and in the absence of a severe medical condition. These results could help guide clinical and public-health decisions regarding clozapine blood monitoring guidelines.

Objective: To evaluate trends in psychiatric bed occupancy by elderly inpatients in the Brazilian public health care system between 2000 and 2010 and to determine the leading psychiatric diagnosis for hospital admissions. Methods: Data from all 895,476 elderly psychiatric admissions recorded in the Brazilian Public Health Care Database (DATASUS) between January 2000 and February 2010 were analyzed. Polynomial regression models with estimated curve models were used to determine the trends. The number of inpatient days was calculated for the overall psychiatric admissions and according to specific diagnoses. Results: A moderate decreasing trend (p < 0.001) in the number of inpatient days was observed in all geriatric psychiatric admissions (R2 = 0.768) and in admissions for organic mental disorders (R2 = 0.823), disorders due to psychoactive substance use (R2 = 0.767), schizophrenia (R2 = 0.680), and other diagnoses (R2 = 0.770), but not for mood disorders (R2 = 0.472). Most admissions (60 to 65%) were due to schizophrenia. Conclusion: There was a decreasing trend in inpatient days for elderly psychiatric patients between 2000 and 2010. The highest bed occupancy was due to schizophrenia, schizotypal, and delusional disorders.

Objetivo Descrever o caso de um paciente com esquizofrenia em tratamento com clozapina acometido por um episódio de heat stroke. Descrição do caso Durante uma onda de calor em janeiro e fevereiro de 2014, um paciente com esquizofrenia em tratamento com clozapina foi inicialmente encaminhado para diagnóstico diferencial de infecção sistêmica e síndrome neuroléptica maligna, tendo obtido o diagnóstico final de heat stroke, tratado com controle de temperatura corporal e hidratação. Comentários Este relato de caso tem como objetivo alertar os clínicos para este diagnóstico diferencial, que pode surgir com mais frequência à medida que as temperaturas globais continuarem a aumentar, e também destacar a importância da realização de um diagnóstico mais acurado, que possa melhorar as decisões de tratamento e os desfechos clínicos para os pacientes.

Objective To describe the case of a patient with schizophrenia on clozapine treatment who had an episode of heat stroke. Case description During a heat wave in January and February 2014, a patient with schizophrenia who was on treatment with clozapine was initially referred for differential diagnose between systemic infection and neuroleptic malignant syndrome, but was finally diagnosed with heat stroke and treated with control of body temperature and hydration. Comments This report aims to alert clinicians take this condition into consideration among other differential diagnoses, especially nowadays with the rise in global temperatures, and to highlight the need for accurate diagnosis of clinical events during pharmacological intervention, in order to improve treatment decisions and outcomes.

OBJETIVO: Pesquisas sugerem as citocinas como potenciais mediadores da interação entre os sistemas imune e neuroendócrino, e que existe um estado pró-inflamatório associado com transtorno bipolar e esquizofrenia. O objetivo deste estudo é comparar os níveis de citocinas entre os dois distúrbios. MÉTODO: Vinte pacientes com transtorno bipolar eutímicos, 53 pacientes com esquizofrenia cronicamente estabilizados e 80 controles saudáveis foram recrutados. Todos os indivíduos são não-fumantes e não-obesos. As citocinas TNF-α, IL-6 e IL-10 foram examinadas por ELISA sanduíche. RESULTADOS: A IL-6 estava aumentada nos pacientes com esquizofrenia quando comparados aos controles (p < 0,0001) e aos pacientes bipolares eutímicos (p < 0,0001). Os níveis de IL-6 não foram diferentes nos controles em comparação com pacientes com transtorno bipolar eutímicos (p = 0,357). Os níveis de IL-10 foram menores nos controles quando comparados aos esquizofrenia (p = 0,001) ou aos bipolares (p = 0,004). Não houve diferença significativa nos níveis séricos de TNF-α entre os grupos (p = 0,284). A separação por sexo não mostrou diferenças significativas e não houve correlação entre a dose de antipsicóticos e os níveis de citocinas em pacientes com esquizofrenia. DISCUSSÃO: Estes resultados evidenciam uma ativação imune crônica na esquizofrenia. O transtorno bipolar parece apresentar um aumento da atividade inflamatória relacionado ao episódio de humor. Níveis maiores de IL-10 no transtorno bipolar e esquizofrenia sugerem diferentes padrões de equilíbrio inflamatório entre esses dois transtornos. Resultados fornecem apoio adicional para a investigação de citocinas como possíveis biomarcadores para a atividade da doença ou resposta ao tratamento.

OBJECTIVE: Previous reports suggest that cytokines act as potential mediators of the interaction between the immune and neuroendocrine systems, and that a proinflammatory state may be associated with bipolar disorder and schizophrenia. The aim is to compare cytokine levels in both disorders. METHOD: Twenty euthymic bipolar disorder patients, 53 chronic stabilized schizophrenia patients and 80 healthy controls were recruited. Subjects were all non-smokers and non-obese. Cytokines TNF-α, IL-6, and IL-10 were examined by sandwich ELISA. RESULTS: IL-6 levels were increased in schizophrenia patients when compared to controls (p < 0.0001) and euthymic bipolar disorder patients (p < 0.0001). IL-6 levels were no different in controls compared to euthymic bipolar disorder patients (p = 0.357). IL-10 was lower in controls compared to schizophrenia patients (p = 0.001) or to bipolar disorder patients (p = 0.004). There was no significant difference in TNF-α serum levels among the groups (p = 0.284). Gender-based classification did not significantly alter these findings, and no correlation was found between the antipsychotic dose administered and cytokine levels in patients with schizophrenia. DISCUSSION: These findings evidence a chronic immune activation in schizophrenia. Bipolar disorder seems to present an episode-related inflammatory syndrome. Increased anti-inflammatory factor IL-10 in bipolar disorder and schizophrenia suggests different patterns of inflammatory balance between these two disorders. Results further support the need to investigate cytokines as possible biomarkers of disease activity or treatment response.

OBJETIVO: Avaliar o estado nutricional de pacientes com esquizofrenia, atendidos por um programa de reabilitação social (CAPS). MÉTODOS: Foi realizado um estudo transversal com 40 pacientes com diagnóstico de esquizofrenia, em uso de antipsicóticos, atendidos no CAPS do Hospital de Clínicas de Porto Alegre. Foram verificados medidas antropométricas (peso, estatura, circunferência abdominal, percentual de gordura corporal), pressão arterial e tabagismo. RESULTADOS: A amostra constitui-se de 65% de homens. A média de peso encontrada foi de 75,39 ± 15,73 kg. O índice de massa corporal médio apresentou-se dentro dos parâmetros de sobrepeso (26,76 ± 4,78 kg/m²), sendo 55% dos pacientes com sobrepeso ou obesidade segundo a classificação da OMS/1998. A circunferência abdominal e o percentual de gordura corporal apresentaram-se elevados na maioria dos pacientes (62,5% e 92,5%, respectivamente). Adicionalmente, não houve associação significativa entre IMC e a classificação de antipsicóticos (típicos, atípicos, clozapina). Encontrou-se uma correlação entre o tempo de doença com o percentual de gordura (r = 0,39, p = 0,033) e escolaridade com o peso (r = 0,362, p = 0,046) e IMC (r = 0,372, p = 0,039). Na regressão linear, 13% da variabilidade do percentual de gordura foi explicada pelo tempo de doença (r² = 0,131, B = 0,233, p = 0,049); 13% da variação do peso foi explicada pela escolaridade (r² = 0,131, B = 1,415, p = 0,046) e 13,8% da variação do IMC foi explicada pela escolaridade (r² = 0,138, B = 0,411, p = 0,039). CONCLUSÃO: Os pacientes apresentaram níveis aumentados de circunferência abdominal, percentual de gordura corporal e peso. Aparentemente, o ganho de peso ocorre em todos os pacientes expostos a antipsicóticos, independentemente do tipo de medicação e de resposta clínica, e a qualquer momento ao longo da evolução da doença. Sugere-se que, adicionalmente, a avaliação dos hábitos alimentares e o acompanhamento nutricional desses pacientes, a detecção precoce de alterações associadas à exposição aos antipsicóticos, em especial a obesidade, e o registro de mudanças ao longo do curso da doença e perante exposição a diferentes apresentações, tipos e doses de antipsicóticos sejam essenciais para o entendimento mais preciso do ganho de peso.

OBJECTIVE: To evaluate the nutritional status of patients with schizophrenia attended in a social rehabilitation program (CAPS). METHODS:Cross-sectional study with 40 patients diagnosed with schizophrenia, in antipsychotic use, attended in CAPS of the Hospital de Clínicas de Porto Alegre. Anthropometric measures were verified (weight, height, waist circumference, body fat percentage), blood pressure and cigarette smoking. RESULTS: The sample consisted of 65% of men. Mean of weight was 75,39 ± 15,73 kg and mean body mass index was within the parameters of overweight (26,76 ± 4,78 kg/m²), 55% of the patients with overweight or obesity according to the OMS/1998 criteria. Waist circumference and body fat percentage were high in most patients (62.5% and 92.5%, respectively). In addition, there was no significant association between BMI and antipsychotics drugs (typic and atypic, clozapine). We found a correlation between duration of disease with body fat percentage (r = 0.39, p = 0.033) and education correlated with weight (r = 0.362, p = 0.046) and BMI (r = 0.372, p = 0.039). In linear regression 13% of the variability in fat percentage was explained by disease duration (r² = 0.131, B = 0.233, p = 0.049), 13% of weight variation was explained by education (r² = 0.131, B = 1.415, p = 0.046) and 13.8% of the variation in BMI was explained by education level (r² = 0.138, B = 0.411, p = 0.039). CONCLUSION: Patients showed increased levels of waist circumference, body fat percentage and overweight. Apparently, the weight gain occurs in all patients exposed to antipsychotics, independent from type of drug and clinical response, and at any moment along illness evolution. It is suggested that in addition to food habits an nutritional assessment and follow-up, the clinician should take notes about early changes along the course of illness, changes of type and dose of drugs.

Introdução: A análise dos resultados da expansão da rede de atenção à saúde mental para a população brasileira tem utilizado como medida de desfecho o indicador de cobertura dos Centros de Atenção Psicossocial (CAPS), que considera adequada a existência de um CAPS para cada 100 mil habitantes. O Rio Grande do Sul encontra-se classificado em terceiro lugar no ranking nacional, com índice de 0,7 CAPS/100.000 habitantes. Este estudo objetivou testar a variabilidade de cobertura de cada região para verificar se esse índice global representava as realidades regionais. Método: Foram utilizados dados do Cadastro Nacional de Estabelecimentos de Saúde (CNES) e dados populacionais do ano de 2009 disponibilizados pelo site do Departamento de Informática do Sistema Único de Saúde do Ministério da Saúde. Foram calculados os indicadores de cobertura para as 19 regionais de saúde, sendo gerado índice de cobertura resultante da razão entre o indicador observado e o esperado de acordo com a população de cada região geográfica analisada. Resultados: Analisada a variabilidade de cobertura de cada região, foi evidenciada a ocorrência de sete regiões com cobertura insuficiente, representando 49% da população do estado com cobertura inadequada. Conclusão: O estudo demonstrou que o uso do índice global é falacioso, pois não representa as realidades regionais, sendo que cerca de metade das regiões com excesso de cobertura mascaram as regiões deficientes. Isso sugere que as análises de cobertura devem ser realizadas por áreas geográficas para identificar carências regionais e fornecer subsídios para a extensão da rede de forma igualitária para usuários de diferentes regiões do estado.

Introduction: Analysis of the results of the ongoing expansion of the Brazilian public mental health care network has used the indicator of coverage offered by Psychosocial Care Centers (Centros de Atenção Psicossocial, CAPS), which considers the rate of one CAPS per 100,000 inhabitants to be adequate, as its outcome measure. The state of Rio Grande do Sul ranks third in nationwide CAPS coverage standings, with 0.7 centers per 100,000 inhabitants. The present study sought to assess the variability of coverage in different regions in order to verify the representativeness of the overall coverage rate. Method: We used data collected from the National Database of Health Facilities (Cadastro Nacional de Estabelecimentos de Saúde, CNES) and 2009 population data made available at the website of the Ministry of Health Department of Information Technology. We calculated 19 coverage indicators, one for each health region, and computed a coverage rate based on the ratio between the actual indicator and the expected indicator according to the population of each region. Results: Analysis of the variability of coverage for each region showed that seven regions had inadequate coverage, with 49% of the state population receiving inadequate coverage. Conclusion: This study showed that the use of an overall rate for mental health care coverage can be considered a fallacy, as it fails to represent regional realities; roughly half of the regions with excessive coverage masked the regions with poor coverage. This finding suggests that analysis of care coverage must be broken down into geographic areas, in order to identify regional needs and provide support for equal expansion of the community mental health care network for users living in different geographic areas.

OBJETIVO: A eficácia da eletroconvulsoterapia em tratar sintomas depressivos está estabelecida por meio de inúmeros estudos desenvolvidos durante as últimas décadas. A eletroconvulsoterapia é o tratamento biológico mais efetivo para depressão atualmente disponível. O objetivo deste estudo foi demonstrar o papel da eletroconvulsoterapia no tratamento da depressão e destacar aspectos atuais relativos à sua prática. MÉTODO: Foram revisados na literatura estudos de eficácia, remissão de sintomas, fatores preditores de resposta, assim como aspectos atuais acerca da qualidade de vida, percepção dos pacientes, mecanismo de ação, técnica e prejuízo cognitivos. RESULTADOS: Os principais achados desta revisão foram: 1) a eletroconvulsoterapia é mais efetiva do que qualquer medicação antidepressiva; 2) a remissão da depressão com a eletroconvulsoterapia varia, em geral, de 50 a 80%; 3) Ainda é controverso o efeito da eletroconvulsoterapia nos níveis de fator neurotrófico derivado do cérebro (acho que aqui pode colocar entre parenteses o "BNDF"); 4) a eletroconvulsoterapia tem efeito positivo na melhora da qualidade de vida; 5) os pacientes submetidos à eletroconvulsoterapia, em geral, têm uma percepção positiva do tratamento. CONCLUSÃO: A eletroconvulsoterapia permanece sendo um tratamento altamente eficaz em pacientes com depressão resistente. Com o avanço da sua técnica, a eletroconvulsoterapia tornou-se um procedimento ainda mais seguro e útil tanto para a fase aguda, quanto para a prevenção de novos episódios depressivos.

OBJECTIVE: The efficacy of electroconvulsive therapy in treating depressive symptoms has been established by means of innumerable studies developed along the last decades. Electroconvulsive therapy is the most effective biological treatment for depression currently available. The objective of this study was to demonstrate the role of electroconvulsive therapy in the treatment of depression and highlight present aspects related to its practice. METHOD: We reviewed in the literature studies on efficacy, symptom remission, predictive response factors as well as current aspects regarding quality of life, the patients' perception, mechanism of action, technique and cognitive impairment. RESULTS: The main results found in the this revision were: 1) electroconvulsive therapy is more effective than any antidepressant medication; 2) the remission of depression with electroconvulsive therapy varies, in general, from 50 to 80%; 3) The effect of electroconvulsive therapy in brain-derived neurotrophic factor levels is still controversial; 4) electroconvulsive therapy has a positive effect in the improvement of quality of life; 5) patients submitted to electroconvulsive therapy have, in general, a positive perception about the treatment. CONCLUSION: Electroconvulsive therapy remains a highly efficacious treatment in treatment-resistant depression. With the improvement of its technique, electroconvulsive therapy has become an even safer and more useful procedure both for the acute phase and for the prevention of new depressive episodes.

OBJETIVO: As neurotrofinas, enzimas antioxidantes e marcadores de oxidação têm interações. Este estudo verificou se existem correlações entre os níveis séricos de superóxido-dismutase, uma enzima chave na defesa antioxidante, os produtos de reação com o ácido tiobarbitúrico, um indicador direto de peroxidação lipídica, e o fator neurotrófico derivado do cérebro, a neurotrofina mais amplamente distribuída. MÉTODO: Sessenta pacientes portadores de Esquizofrenia pelo DSM-IV foram incluídos (43 homens, 17 mulheres), com idade média de 34,7 ± 10,8 anos, idade média no primeiro episódio de 19,8 ± 7,9 anos, e tempo médio de duração da doença de 14,9 ± 8,5 anos. Foi coletado sangue de cada sujeito para a determinação dos níveis séricos de fator neurotrófico derivado do cérebro, superóxido-dismutase e ácido tiobarbitúrico. RESULTADOS: Os níveis de fator neurotrófico derivado do cérebro se correlacionaram positivamente aos de ácido tiobarbitúrico (r = 0,333, p = 0,009) e não mostraram correlação com os de superóxido-dismutase (r = - 0,181, p = 0,166). Este último também não se correlacionou aos níveis de ácido tiobarbitúrico (r = 0,141, p = 0,284). CONCLUSÕES: A correlação positiva entre fator neurotrófico derivado do cérebro e ácido tiobarbitúrico direciona para investigações na interação intracelular entre neurotrofinas, enzimas antioxidantes e marcadores de oxidação, além de abrir perspectives para pesquisa em tratamentos para a prevenção da neurotoxicidade ao longo do curso da esquizofrenia.

OBJECTIVE: The neurotrophins, antioxidant enzymes and oxidative markers have reciprocal interactions. This report verified in chronically stable medicated schizophrenic patients whether there are correlations between the serum levels of superoxide dismutase, a key enzyme in the antioxidant defense, thiobarbituric acid reactive substances, a direct index of lipid peroxidation, and brain-derived neurotrophic factor, the most widely distributed neurotrophin. METHOD: Sixty DSM-IV schizophrenic patients were included (43 males, 17 females). Mean age was 34.7 ± 10.8 years, mean age at first episode was 19.8 ± 7.9 years, and mean illness duration was 14.9 ± 8.5 years. Each subject had a blood sample collected for the determination of serum levels of brain-derived neurotrophic factor, thiobarbituric acid reactive substances and superoxide dismutase. RESULTS: Brain-derived neurotrophic factor levels showed a positive correlation with thiobarbituric acid reactive substances levels (r = 0.333, p = 0.009). Brain-derived neurotrophic factor levels were not correlated with superoxide dismutase levels (r = - 0.181, p = 0.166), and superoxide dismutase levels were not correlated with thiobarbituric acid reactive substances levels (r = 0.141, p = 0.284). CONCLUSIONS: The positive correlation between brain-derived neurotrophic factor and thiobarbituric acid reactive substances suggests the need of further investigation on intracellular interactions of neurotrophins, antioxidant enzymes and oxidative markers. In addition, this opens a venue for investigation on treatments for the prevention of neurotoxicity along the course of schizophrenia.

O presente artigo tem como objetivo sintetizar estudos sobre os testes neuropsicológicos que estão sendo mais utilizados em esquizofrenia. Foi realizada uma revisão sistemática da literatura, consultando-se: PubMed, MEDLINE (via PubMed) e LILACS, no período entre 1995 e 2006. As palavras-chave foram: cognition and schizophrenia; evaluation and cognition and schizophrenia; functioning and cognition and schizophrenia. Foi feita uma pesquisa manual de referências dos artigos mais citados, incluindo estudos de metanálise, revisão teórica e da Measurement and Treatment Research to Improve Cognition in Schizophrenia. Critérios de inclusão: estudos com pacientes com esquizofrenia que mencionaram a utilização de testes neuropsicológicos. Foi feita uma revisão sobre os testes mais citados, para verificação de nomenclatura, indicações e quais estavam validados para o Brasil. Foram identificados 98 estudos: 77 empíricos, oito metanálises, seis revisões teóricas e sete da Measurement and Treatment Research to Improve Cognition in Schizophrenia. Foram encontrados 102 diferentes nomes de testes neuropsicológicos, com citação de 10 baterias de testes, na maioria compostas por subtestes já padronizados. Entre os 77 estudos empíricos, 22 (28,57%) foram sobre validação de instrumentos ou baterias de testes. Entre os 10 testes mais citados, apenas três possuem validação para o Brasil (WCST; Wechsler Adult Intelligence Scale; Mini-Mental Status Examination). Esses achados evidenciam uma ampla heterogeneidade nos critérios de seleção dos testes neuropsicológicos, divergências na nomenclatura e certa tendência para a criação de novos testes e baterias para avaliação de funções cognitivas. A relevância e características dos prejuízos cognitivos na esquizofrenia revelam a urgente necessidade de identificação específica e padronizada de métodos de avaliação neuropsicológica para pacientes brasileiros.

This article aims at summarizing studies on the most commonly used neuropsychological tests in schizophrenia. A systematic literature review was performed on PubMed, MEDLINE (via PubMed) and LILACS, from 1995 through 2006. The keywords were cognition and schizophrenia; evaluation and cognition and schizophrenia; functioning and cognition and schizophrenia. A manual research of references in most frequently cited articles was performed, including meta-analysis studies, theoretical reviews, and the Measurement and Treatment Research to Improve Cognition in Schizophrenia. Inclusion criteria were studies on schizophrenia patients that reported use of neuropsychological tests. A review of the most frequently cited tests was performed to check nomenclature, indications, and which were validated for Brazil. A total of 98 studies were identified: 77 empirical studies, eight meta-analyses, six theoretical reviews and seven from the Measurement and Treatment Research to Improve Cognition in Schizophrenia. There were 102 different names of neuropsychological tests; 10 batteries of tests were cited, most of them composed by previously standardized subtests. Of the 77 empirical studies, 22 (28.57%) were about validation of instruments or batteries of tests. Of the 10 most frequently cited tests, only three had been validated for Brazil (WCST; Wechsler Adult Intelligence Scale; Mini-Mental Status Examination). These findings show a wide heterogeneity in the selection criteria of neuropsychological tests, disagreement as to the nomenclature, and a certain trend toward creation of new tests and batteries for evaluating cognitive functions. The relevance and characteristics of cognitive impairments in schizophrenia confirm the urgent necessity of specific and standardized identification of neuropsychological evaluation methods for Brazilian patients.
